How it works:
1. Contact us by phone or by email
2. We will send you a brief intake document along with a required list of documents we will need from you
3. Once we have received the documentation, we will contact you to book an appointment as a Centre closest to you or via telemedicine
4. Our Nurse educators will meet you on-site at your appointment date/time and work with you through the process
